What is Aftercare?

Post-rehab aftercare is the treatment a person will receive after they have taken part in a drug or alcohol rehab program. It has to do with any ongoing or follow-up treatment for substance use which takes place after the initial rehab program.

Aftercare programs have been shown to be quite effective in helping people stick to their sobriety, lowering the risk of relapse.

Whatever the setting or treatment provider, an aftercare program will aim to:

Outpatient Treatment Services

A common form of aftercare involves outpatient services which allow patients to live at home while still receiving proper addiction treatment. An individual treatment plan is designed specifically for the patient’s needs and schedule. They offer peace of mind by helping patients overcome addiction while maintaining a home life. 

Patients who are likely to benefit from outpatient treatment programs have a less severe addiction and lower risk of serious withdrawals. They also have a strong support system and reliable transportation.

Continuing Case Management

Continuing case management is a process that manages client wellness. This is done using communication, education, and assisting of services.

Expert case managers coordinate all aspects of on-going aftercare once the patient is discharged. The case manager will organize all resources in the client’s home area. They work closely with families to educate them on the client’s options and ensure they have the appropriate care plan.

Case managers are constantly networking and building relationships to ensure the patient is going home with proper resources and services.

Sober Living Environment

A sober living environment is designed to help individuals begin their transition to drug-free living. Clients live in ‘sober’ residences with others who are recovering from substance abuse and/or other issues. It is also for transitioning individuals who have completed their inpatient rehabilitation program. 

The community offers some freedoms, along with daily responsibilities. A sober living house is safe, comfortable, and structured. Depending on the needs of the client, a sober residence can range from minimal to a highly structured environment.

Sober Coaches and Mentors

Sober coaches and mentors are professionals who work with individuals to achieve sobriety. They are experienced, highly supportive colleagues who provide virtually unlimited support, guidance and help to those in the recovery process. Some responsibilities include keeping schedules for their clients and collaborating with other additional treatment options. They give emotional and psychological support and are highly skilled in crisis management and interventions. 

Sober coaching is meant to be temporary. While some individuals might need coaching for a few weeks, others may need months or years. Sober coaches focus on long-term views so that the client can prolong their sobriety, maintain recovery, and achieve normal living. 

It is important that a patient has chemistry with their mentor. They are available to speak on a regular basis and meet as often as you are comfortable with. They are also available under any circumstance of an emergency.

Support Groups

Treatment centers offer a variety of support groups and aftercare programs. They are designed to help individuals stick to their goals and maintain sobriety. These gatherings or meetings are composed of people in similar situations and happen on a regular basis.

The best way to achieve success in the first year of recovery is to attend support group meetings regularly. These groups provide a new lifestyle, accountability, structure, as well as support.

  • 12-Step Recovery Programs (Anonymous Groups)
  • Non-12 Step Recovery Programs (SMART Recovery)

An aftercare program can greatly help with learning to cope with returning to “normal life” without returning to addictive behaviors and substances. Through aftercare, people in recovery also discover how to involve their friends and families in their recovery, adding additional support.
